Onze praktijk is geopend voor mensen die wonen op de volgende postcodes:

U kunt zich bij ons inschrijven door langs te komen op de praktijk met uw legitimatiebewijs en een ondertekend inschrijfformulier.

Let op: U kunt pas een afspraak voor het spreekuur maken als wij het dossier van uw vorige huisarts hebben ontvangen. Het is erg belangrijk dat u uw vorige huisarts meldt dat u zich bij hen uitschrijft.

You can register with us by visiting the practice with your ID and a signed registration form.

Please note:If you were subscribed by a dutch GP before: You can only make an appointment for the consultation hour once we have received the file from your previous GP. It is very important that you inform your previous GP that you are deregistering from them.
